OCD Medication Management | Northern Virginia Psychiatry

You shouldn’t have to explain what OCD is to your own prescriber.

OCD treatment here means medication dosed for OCD specifically, working alongside your ERP therapy, with the obsession content met without a flicker of reaction.

What is OCD, clinically?

Obsessions are unwanted, distressing thoughts, images or urges. Compulsions are the behaviours or mental acts done to reduce that distress, including checking, washing, counting, mental reviewing and reassurance seeking. The relief is brief, which is what makes the cycle strengthen over time.

Almost everyone has intrusive thoughts. In OCD they stick, cause real distress, and drive compulsions, which is what OCD treatment is built around.

The volume of distress and the compulsion are what separate OCD from ordinary intrusive thinking.

Why does OCD need different medication dosing?

OCD commonly requires higher doses and a longer trial period than depression before response is judged. Treatment stopped too early or at too low a dose is a frequent reason people conclude OCD medication doesn’t work for them. Specific dosing is always an individual clinical decision.

This is the single most useful thing to know before starting OCD treatment, and it’s the thing most often missed when OCD gets folded into general anxiety.

Expect the trial to take longer than you’d like. Slow response is a feature of OCD treatment rather than a sign it isn’t working.

How ERP and medication work together

  • ERP is the leading psychological treatment for OCD.
  • It helps you face triggers without compulsions, breaking the OCD cycle.
  • Medication can reduce symptoms and support ERP.
  • Medication is a support, not a standalone cure.
  • Working with an ERP therapist and prescriber can improve treatment.

The goal is to respond differently to OCD and regain control of daily life.

A sample case study

An Illustrative Example. Not a real person.

A 26-year-old in Centreville has had harm-themed intrusive thoughts since university. She has never told anyone the content, including 2 previous providers who diagnosed generalised anxiety.

She was prescribed a standard antidepressant dose. It helped her anxiety slightly and did nothing for the compulsions, which now take about 90 minutes a day.

The evaluation asks directly about themes, rituals and mental compulsions. The diagnosis shifts. So does the dosing strategy and the trial length she’s told to expect.

She starts ERP therapy in parallel with the OCD treatment. At 4 months the rituals are down to roughly 15 minutes a day.
