Appointments and Cancellations

Appointments are scheduled in advance based on treatment needs, goals, and mutual availability. When an appointment is scheduled, that time is reserved specifically for you.

If you need to cancel or reschedule, please provide at least 24 hours notice. Appointments missed or canceled with less than 24 hours notice may result in a $100 late-cancellation or no-show fee because the reserved time generally cannot be filled on short notice.

If an applicable insurance plan or payer contract prohibits a late-cancellation or no-show fee, the fee will not be charged. Late-cancellation and no-show fees are not submitted to insurance.

Applicable unpaid administrative or cancellation fees may need to be resolved before additional appointments are scheduled. Repeated missed appointments or late cancellations may result in discharge from the practice after appropriate notice and continuity-of-care planning.

Billing, Insurance, and Financial Responsibility

Rituality Wellness may bill insurance directly or may use a third-party billing platform, including Headway, depending on the patient’s insurance plan and the practice’s network arrangement.

• Insurance eligibility or benefit checks are provided as a courtesy and are not a guarantee of payment or coverage.

• You are responsible for confirming that the practice and provider are in network with your plan and for understanding your deductible, copayment, coinsurance, referral, prior-authorization, and other benefit requirements.

• You agree to provide complete and current insurance information and to notify the practice promptly of coverage changes.

• You authorize Rituality Wellness and its billing partners to submit claims and to use or disclose the minimum information reasonably necessary for treatment, payment, and health care operations as permitted by law.

• You request that applicable insurance benefits be paid to Rituality Wellness or its authorized billing entity when assignment of benefits is permitted.

• You are responsible for patient-responsibility amounts shown by your insurer, including deductibles, copayments, coinsurance, noncovered services, and amounts resulting from inaccurate or inactive coverage information, subject to applicable law and payer contracts.

Headway-Billed Services

When covered services are billed through Headway, claims and patient-responsibility amounts may be processed through the Headway platform. Patients using Headway are responsible for maintaining an active payment method and for addressing billing questions through Headway when the issue relates to Headway claim or payment processing.

Direct-Billed Services

When services are billed directly by Rituality Wellness, applicable patient-responsibility balances may be charged to the payment method on file in accordance with the signed Payment Card Authorization after insurance processing or according to an agreed self-pay arrangement.

Government and Other Payer Requirements

The practice will follow applicable federal and state billing requirements and payer contracts. A request to self-pay or not submit a claim cannot be honored when law or a payer requirement requires claim submission or otherwise limits private payment arrangements.

Self-Pay Services and Good Faith Estimates

Patients who do not have insurance or who choose not to use insurance, when permitted, may be asked to sign a Self-Pay Election and Financial Responsibility Agreement. Eligible uninsured or self-pay patients will receive a Good Faith Estimate when required by federal law. Current self-pay rates and administrative fees are provided separately and may be updated prospectively.

Administrative and Noncovered Services

Some services are not typically covered by insurance and may be billed separately. Examples may include:

• Late-cancellation or missed-appointment fees as described above

• Forms, letters, disability paperwork, or other written documentation completed outside a scheduled appointment

• Extended clinical telephone calls or clinical work outside a scheduled appointment when separately billable

• Medical-record copy fees to the extent permitted by applicable law

• Other agreed administrative services not included in a covered visit

When a separate fee applies, the practice will make reasonable efforts to inform you of the fee before completing the requested service.

Patient Portal, Electronic Communication, and Telehealth Platform

Rituality Wellness uses PracticeQ as its electronic health record and patient portal for appointment management, forms, documentation, prescribing, secure messaging, and telehealth access. Patients should use the PracticeQ portal for non-urgent clinical communication whenever possible.

Telehealth visits are conducted through the secure platform or link designated by Rituality Wellness. Automated messages or links from another platform may relate to billing or administrative services and do not necessarily indicate where the clinical visit will occur.

Email, standard text messaging, and voicemail may have privacy limitations. Do not use routine messaging for emergencies or time-sensitive safety concerns.

Availability and Emergencies

Rituality Wellness is an outpatient psychiatric practice and does not provide 24-hour emergency or crisis services. Routine messages are reviewed during normal business hours, and responses may take up to one to two business days.

If you are experiencing a psychiatric emergency, suicidal thoughts, thoughts of harming yourself or another person, or another urgent safety concern, call 911, go to the nearest emergency department, or call or text 988 for the Suicide & Crisis Lifeline.

Prescriptions and Controlled Substances

Medication refills and controlled-substance prescribing require appropriate follow-up and monitoring. Patients receiving controlled substances may be required to sign and periodically renew a separate Controlled Substances Treatment Agreement.

Ending or Transferring Care

The treatment relationship may end when you choose another provider, when a different level of care is recommended, when treatment is no longer clinically appropriate within this practice, or when practice policies are repeatedly not followed. Discharge may also occur for repeated missed appointments, persistent nonpayment of applicable balances, unsafe medication practices, or persistent abusive or threatening communication.

When clinically appropriate and reasonably possible, discharge will be discussed in advance and written notice, medication transition planning, and referral resources will be provided to support continuity of care.
